Continuum of Care covers the delivery of healthcare over a period of time, and may refer to care provided from birth to end of life. Healthcare services are provided for all levels and stages of care.
- Continuum Healthcare Network includes educational outreach programs and events, wellness and health promotions, prevention, care coordination, patient advocacy, condition management and complex case management.
- Continuum Healthcare Network promotes the role of these and other strategies in raising care quality, improving health outcomes and reducing preventable health care costs for individuals with or at risk of chronic disease.
- Continuum Healthcare Network helps healthcare providers to reduce the rate of their readmissions and health care cost through advocacy, research, community educational outreach programs, event collaboration, networking, forums and the promotion of best practices.

What are Advance Directives and Why Are They Important?
What happens if you experience a catastrophic accident or illness that leaves you unconscious or unable to communicate? How do you make sure your wishes about your medical care are known? Having an advance directive is the answer.
There are many places to get forms, including medical, religious, aging assistance, and legal organizations. Three places are shown below, but these are just examples. Any of these forms are valid in Maryland, but not all may be in keeping with your beliefs and values. Your advance directive does not have to be on any particular form.

CHN Magazine highlights stories of success, innovation, programs, and promotes community support of local services and programs. Continuum Healthcare Network Magazine (CHNM) prints four issues a year and circulates more than 10,000 hard copies. Continuum Healthcare Network magazine is circulated and distributed in the DMV metro area. We distribute them to the hospitals, urgent care, adult daycare, assisted living, independent living, senior recreational facilities, physicians' offices, in-home care offices, pharmacies, grocery stores, schools, and to CHNM members and emailed to our community of 30,000 plus households.
