Continuum Healthcare Network focused on providing a comprehensive, coordinated system designed to meet the needs of individuals with complex and/or ongoing problems in an efficient and effective manner. 

  

Continuum Healthcare Network services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, continuum healthcare network physicians, nurse practitioners, healthcare professionals and other providers are teamed with outreach organizations, staff, case managers, social workers and substance abuse counselors to provide a well-rounded continuum of care.

Continuum Healthcare Network  covers the delivery of healthcare over a period of time and may refer to care provided from birth to end of life. Healthcare services are provided for all levels and stages of care.

Our Programs and Services

Mission: Continuum Healthcare Network's Asian American and Pacific Islander Data Representation Initiative for Alzheimer's and Dementia At Continuum Healthcare Network, our mission is to improve the health and well-being of all individuals in our community. Recognizing the importance of equitable representation and inclusive healthcare practices, we are launching the Asian American and Pacific Islander Data Representation Initiative for Alzheimer's and Dementia.

This initiative aims to address the disparities in data representation and healthcare outcomes for Asian American and Pacific Islander communities affected by Alzheimer's and Dementia. We aim to bridge the information gap and promote culturally sensitive and targeted care to ensure that individuals from these communities receive appropriate support and resources.

Key Goals:

  1. Collect Comprehensive Data: We will work collaboratively with community organizations, healthcare providers, researchers, and advocacy groups to collect comprehensive data on Alzheimer's and Dementia among Asian American and Pacific Islander populations. This will involve assessing prevalence, risk factors, caregiving challenges, access to resources, and disparities in diagnosis and treatment.
  2. Promote Cultural Competence: We will develop educational programs and training initiatives to enhance cultural competence among healthcare professionals, caregivers, and service providers. By fostering understanding and awareness of the unique cultural nuances, values, and practices within Asian American and Pacific Islander communities, we aim to improve communication, engagement, and the overall quality of care provided.
  3. Advocate for Policy Changes: We will collaborate with policymakers, government agencies, and influential stakeholders to advocate for policy changes that address the needs and challenges faced by Asian American and Pacific Islander communities affected by Alzheimer's and Dementia. Our goal is to ensure that healthcare policies and programs are inclusive, equitable, and tailored to meet the specific requirements of these communities.
  4. Increase Awareness and Outreach: We will launch targeted awareness campaigns to promote early detection, risk reduction strategies, and available resources for Alzheimer's and Dementia within Asian American and Pacific Islander communities. These efforts will include community workshops, culturally appropriate materials, language access, and partnerships with local organizations to facilitate outreach and engagement.
  5. Foster Research and Innovation: We will support and facilitate research endeavors focused on understanding the unique genetic, environmental, and socio-cultural factors contributing to Alzheimer's and Dementia among Asian American and Pacific Islander populations. By encouraging research collaboration, we aim to generate knowledge, identify effective interventions, and develop innovative approaches to improve outcomes and quality of life for individuals affected by these conditions.

Through the Asian American and Pacific Islander Data Representation Initiative for Alzheimer's and Dementia, "Continuum Healthcare Network is committed to eliminating disparities, promoting culturally sensitive care, and empowering communities to lead healthier lives. Together, we can create a future where everyone, regardless of their background, receives the care and support they deserve".

Behavioral Health Integration and Cognitive Rehabilitation

    It is a great honor to invite you to be our guest and honor you to our gala celebrating "Women's History Month," Her Story, Her Voice, Her Love, Her Power. We are celebrating the accomplishments and contributions of women to our history.

    March is Women's History Month, and this year is another historic year for the state of Maryland. We elected the first female and Asian Lieutenant Governor, the first Asian Secretary of State, and the first woman Comptroller. This is a remarkable year in our history and the continuous progress of women's excellence.

    More than ever, this is the most important year to recognizing the achievements of women in all facets of life – education, community, healthcare, science, government, literature, arts, sports, and medicine – which has a huge impact on the development of self-respect and new opportunities for girls, young women and particularly to the Asian American and Pacific Islander Community.

    On Saturday, March 25, 2023, at Silver Spring Civic Building from 5:30 PM-8:30 PM at 1 Veterans Plaza, Silver Spring, MD 20910. We want to recognize and honor you for your accomplishments and for being an icon for women making history. We are celebrating and honoring women for their achievements and contributions to our history.

    Our history is our strength. In our personal lives, we are encouraged by the stories about our mothers, grandmothers, and great-grandmothers to help us better understand their lives, the challenges they faced, and ultimately, ourselves and our times. Recognizing the dignity and accomplishments of women in our own families and those from other backgrounds leads to higher self-esteem among girls and greater respect among boys and men.

    We draw strength and inspiration from those who came before us – and those remarkable women working among us. They are part of our story, and a truly balanced and inclusive history recognizes how important women have always been in our society. The legacy of how others shaped the community sparks our longing to contribute. Everyone needs role models – footsteps enough like our own to inspire us. We also want to include your story in our program to inspire more women. We are looking forward to your presence.
